with a last name of "Smith" and a first name beginning
with "J."
Notes:
The "Last Name" and "License Number" search fields display a licensee's Mailing
City. The "City" option searches a licensee's Work or Business
City.
Records or disciplinary actions prior to 1994 may not be displayed, please
contact the Board directly.
